Oak Creek-Franklin Joint School District

COPPA Parental Consent Form

The Children’s Online Privacy Protection Act (COPPA) is a federal law that safeguards the online privacy of children.  It requires website and online service operators to obtain verifiable parental consent before collecting personal information from children under 13 years old.

COPPA permits schools to act as agents for parents and provide consent to collect personal information only when it is used for educational purposes.

The Oak Creek-Franklin School District utilizes various online tools and educational applications - such as Clever and other Curriculum Department-approved third-party sites - to support and enhance students’ learning.  These platforms may collect limited personal information (such as name, grade level, and login data) to:

  • Create individual student accounts

  • Track academic progress

  • Deliver personalized learning experiences throughout the school year

Please fill out the consent and verification below to indicate whether you give consent for Oak Creek-Franklin Joint School District to collect, use, and disclose your child’s information.
