After submitting your application in Step 1 below, bring the required documents found in Step 2
to the District Office, March 15-19, to complete your child's enrollment for the 2021-2022 school year.
There are two steps to complete the enrollment process for your new student. This process is for residents of the Oak Creek-Franklin Joint School District and families who have received notice of acceptance through the Open Enrollment program.

5K enrollment is only for students NOT currently enrolled in the District (not currently attending the District's 4K program).

To finish the enrollment process, parents/guardians need to bring documents to District Office.
What to bring:
- Your photo ID
- Your child’s original birth certificate (no copies)
- A printed copy of your submitted online enrollment application
- Proof of Residency document (Current WE Energies or Water bill, signed lease agreement, property tax bill)
- Completed Proof of Residency Form
- Completed Records Request Form
- Your child's immunization record (available on the Wisconsin Immunization Registry)
- Completed Immunization Waiver (if applicable; immunization requirements on page 5)
- Completed 4K Questionnaire or 5K Questionnaire (if applicable)
