4K & 5K Enrollment

For the 2019-2020 School Year

There are two steps to complete the enrollment process for your new 4K or 5K student(s) fpr the 2019-2020 School Year:

Please note, this process is for residents of the Oak Creek-Franklin Joint School District only. 5K enrollment is only for students NOT currently enrolled in the District (not currently attending the District's 4K program or Early Childhood program). To enroll in 5-Year-Old Kindergarten (5K), students must be 5 years old on or before September 1, 2019. To enroll in 4-Year-Old Kindergarten (4K), students must be 4 years old on or before September 1, 2019.

Before you begin, please gather the following information:

  • Household Information - address and phone numbers
  • Student Information - health and medication information
  • Parent/Guardian Information - work and cell phone numbers, and email addresses
  • Emergency Contact Information - names and phone numbers

Please note: This process is for residents of the Oak Creek-Franklin Joint School District and non-residents who have been notified that their child has been accepted into the open enrollment program. For questions, call 414-768-5880.


checkmark iconStep 2: Bring Documents to District Office

 

MARCH 11 - MARCH 15, 2019

To finish the enrollment process, parents/guardians need to stop in the District Office to drop off documents during 4K & 5K Enrollment Week: Monday, March 11 - Friday, March 15 from 7:30 - 5:00 p.m.

What to bring:
  • Your photo ID
  • Completed Proof of Residency form
  • Proof of Residency documentation (Current utility bill, signed lease agreement, property tax bill)
  • Your child’s birth certificate
  • Your child's immunization record (Available on the Wisconsin Immunization Registry)
  • Completed 4K Questionnaire OR 5K Questionnaire
  • A printed copy of your completed online enrollment application
